Patient Portal Login - Fallbrook Medical Center

Use our Patient Portal to:

To access your medical information and also be able to attend most of your medical related requests. To access it click on the patient portal login button located in the upper right hand corner of this page.

Once you access your patient portal you will see a list of options to your left where you will have access to view your upcoming appointments, request new appointments, see your lab results, request referrals, send a message to your medical provider and more.

My account: Demographic information such as email, phone number, address.
Messages: send messages to your medical practitioner about anything you may need.
Medical records: access your medical notes, referrals, lab results, medication list and/or any other medical information that you may need to have access to.
Appointments: you will be able to request new appointments and see scheduled appointments already set for you.
