At Fallbrook Medical Center, we strive to provide our patients with a seamless and exceptional experience, recognizing the paramount importance of healthcare and its delivery. If you are a new patient, we kindly ask you to complete the pre-registration form.
If you encounter any issues with the registration process or accessing the patient portal, please don’t hesitate to reach out to one of our representatives for assistance.
If you are a returning patient, please use the Patient Portal for appointment scheduling, billing information, and other medical necessities.
Please bring the following items to
your initial appointment:
- Medical record request authorization
- Make sure to have your insurance card with you. If it’s not accessible, please be ready to cover the entire appointment cost.
- Provide a comprehensive list of your current medications, encompassing prescribed and non-prescribed medications, along with any dietary supplements you’re currently using.
- Your driver’s license or photo ID.
- Your Office Visit copay. (if applicable) This will be indicated on your Insurance ID card.
We recommend arriving at least 30 minutes before your scheduled appointment time period. Please allow extra time for parking and check in process to complete all necessary forms.
If you are coming as a walk-in patient, we strongly advise arriving to our office either at 9:00 AM or 2:00 PM. Remember there are no guaranties you will be seen as a walk-in.
Furthermore, please make arrangements to have your previous medical records sent to us promptly. You can either bring copies with you or request your previous healthcare provider to send them directly to our email at support@fallbrookmedicalcenter.com . To do this, you’ll need to complete the attached medical record request form and send it to your previous doctor’s office.
Lastly, take some time to consider any questions or concerns you’d like to discuss with your doctor. Remember, your Medical provider is your healthcare partner and advocate, and effective communication is crucial for your health success.
