If you encounter any issues with the registration process or accessing the patient portal, please don’t hesitate to reach out to one of our representatives for assistance.
If you are a returning patient, please use the Patient Portal for appointment scheduling, billing information, and other medical necessities.
We recommend arriving at least 30 minutes before your scheduled appointment time period. Please allow extra time for parking and check in process to complete all necessary forms.
If you are coming as a walk-in patient, we strongly advise arriving to our office either at 9:00 AM or 2:00 PM. Remember there are no guaranties you will be seen as a walk-in.
Furthermore, please make arrangements to have your previous medical records sent to us promptly. You can either bring copies with you or request your previous healthcare provider to send them directly to our email at support@fallbrookmedicalcenter.com . To do this, you’ll need to complete the attached medical record request form and send it to your previous doctor’s office.
